- Fill out this form (if applicable*) - Verification of Membership in a NYS EMS Agency DOH-3312
- Submit ALL completed forms with tuition and lab fee to: Colonie EMS 312 Wolf Rd. Latham, NY 12110
- Cash or check/money orders only - we do not process credit cards
